Went to see Dr Forman as a new patient, my wife and I both had an eye exam and he was charging us $100 / each for 6 months contacts prescription. Called my insurance company and they said everything should have been covered without any out of pocket expense. The only thing I would have to pay for is $10 co pay. Luckily, I didn’t purchase contacts today or else I would have been out $200 when it was not necessary. Even tried calling him and explaining to him what my insurance had said and he denied it saying otherwise. Would never go back to this establishment. Dishonest and untrustworthy. ~Sheena
